Audio/Video Request Form

Check the boxes of the items that you want to request and fill out the time, date and NAME boxes. Press the Submit A/V Request button to send your request. Please try togive us at least 24 hours' notice. We will not deny you the equipment and/or service if you don't, but the earlier you notify us, the more likely the service and/or equipment will be available. This is done on a first-come, first-served basis. Thank you.

If you are making your request less than 24 hours' in advance, please come to the Multimedia Studio at Room 5.196 to ensure your request is received properly and your equipment is available. The computer projectors go quickly, so make sure you have it reserved. Most likely, 24 hours' is not enough to ensure availability. If you need to cancel, please call 471-7964. Equipment is in high demand.
